school physicals for 6th grade and new enrollment students will be held on Saturday March, 12, 2016. Appointment letters were mailed the week of Feb. 8th with the time of the appointment. Please verify the appointment time, sign the form and return to the school nurse. This is the only date for school physicals. Failure to attend the physical at the appointed time will result in the student needing to have the exam performed by their primary care provider.
