  • Asthma/Allergy Response Notices and Medications in school

    Asthma/Allergy Response Notices were sent home in Oct. 2015 for those students' who have a history of asthma and/or life threatening allergies.  The purpose of this letter is to update the students' health record with the most current information and to illicit if students need to have medication in school (re: asthma inhaler or epi pen).  

    If you received a letter, please complete it as soon as possible and return it to the school nurse.  If your child needs emergency medication in school, please have your child's health care provider complete the medication form and return it to the school nurse along with the appropriate medication.

    Our location makes it critical that your child has emergency medication in school so he/she can be treated in a timely manner and avoid a trip to the emergency room.  Without your child's medication at school, the only alternative may be to activate the EMS in order to provide the appropriate medical care.

  • Physical examinations

    All 6th grade student and newly enrolled students are required by the State of Pennsylvania, to provide proof of a recent physical exam.  The date of the exam that can be accepted can go back 1 year form the 1st day of 6th grade or 1 year from the date of enrollment.

    If you have not returned the parent permission form indication whether your child will be having a private or school physical exam, please do so as soon as possible.

    For those student who will be having a physical exam done by the school district's physician will receive a letter with the date and time of the exam appointment. Exams are done on a Saturday, usually in March.  Please do not miss your appointment as school exams are only offered once and cannot be rescheduled.  If the appointment is missed, you must have your child examined by a private health care provider.

    If an exam, either school or private, is not on file in the nurse's office by April 30th of the current school year, the student will be excluded from participating in extracurruicular activities, such as clubs and field trips.

  • Immunizations

    The State of Pennsylvania requires that all students are up to date with current immunizations requirements. All students must have the following immunizations:

    All students K-12 are required to have:

    Tetanus: 4 doses with 1 dose on or after the 4th birthday

    Diphtheria: 4 doses with 1 dose on or after the 4th birthday

    Polio: 3 doses

    Measles: 2 doses with the 1st dose after the 1st birthday and 2nd dose after the 4th birthday*

    Mumps: 2 doses with the 1st dose after the 1st birthday and 2nd dose after the 4th birthday*

    Rubella: 1 dose after the 1st birthday*

    Hepatitis B: 3 doses- 3rd dose must be after 6mo. of age

    Varicella: 2 doses

    *Usually given as an MMR

    In addition, 7th grade students must also have a Tdap and Menactra vaccines.

    All requirements are to be submitted to the nurse's office by April 30th of the current school year. Letters are mailed home at regular intervals informing parents of immunization deficiencies. Students may be excluded from attending school effective May 1st of the current school year in the event their immunizations are not up to date.

  • School Physicals March 12, 2016

    School physicals are scheduled for Saturday March 12, 2016.  Appointment letters were mailed home the week of Feb. 8th.  Please complete the form and return to the school nurse.  This will be the only day school physicals will be performed.  Failure to attend the exam at the appointed time will result in the student needing to have the exam performed by their private health care provider.

  • Physician notes

    Recently we have noticed that notes for student illness and injuries from the emergency room or and urgent care center does not include a diagnosis.. This is due to the HIPPA law to protect the students' privacy.  These facilities, for the same reason, cannot provide a diagnosis over the phone without parent permission. It is important to the health and well being of your child that the school nurse be aware of your childs' health status especially in the case of an illness or injury.  Please ask the health provider to supply a diagnosis at the time of the visit to ensure we receive the most current and accurate information.
